What to Expect at Your First Visit
Your first appointment is really about getting to know you. Here’s how to prepare and what will happen — so you can walk in feeling ready, not anxious.
What to bring
- A photo ID
- Your insurance card(s)
- A list of your current medications (with doses, if you have them)
- Any recent lab results, imaging, or records from other doctors
- A referral, if your insurance plan requires one
- Your completed new-patient forms (below)
Save time — fill out your forms first
Completing these ahead of time means less paperwork in the waiting room. Bring them with you, or arrive a few minutes early to fill them out.
What happens during your visit
- Check in. Our front-desk team greets you and confirms your information.
- Meet your provider. They review your health history, your kidney health, and any lab work or records you bring.
- Talk it through. You’ll have time to ask questions and understand what’s going on — in plain language.
- Leave with a plan. Together you’ll agree on next steps and when to follow up.
